This agreement (12-weeks consecutive, 2 classes/wk.) is entered into between Tigermom Aerial Hoop Studio and student purchasing this agreement.

1. Membership Details:

• The Student shall be granted access to attend two classes per week at the Studio.

• The membership duration shall be for a period of 12-weeks consecutive, commencing from the date of this Agreement.

2. Membership Fee:

• The Student agrees to pay a total membership fee of [$199/every 4 weeks (3)= $597] for the entire duration of 12-weeks consecutive.

• The membership fee is to be paid 4-weeks upfront upon signing this Agreement.

3. Termination:

• If the Student wishes to terminate the membership before the completion of the three-month period, a termination fee of $60 shall be applied.

• The termination notice must be provided in writing (email) to the Studio at least [5 days] days prior to the desired termination date.

4. Studio Policies:

• The Student agrees to abide by all Studio rules and policies during the membership period.

• The Studio reserves the right to revoke the membership in case of violation of any Studio policies or disruptive behavior.

5. Liability Waiver:

• The Student acknowledges the inherent risks associated with aerial hoop activities and agrees to release the Studio, its instructors, and staff from any liability for injuries or damages incurred during classes or while on Studio premises.

6. Agreement Binding:

• This Agreement constitutes the entire understanding between the Studio and the Student, superseding any prior agreements or discussions.

• Any amendments or modifications to this Agreement must be made in writing and agreed upon by both parties.

7. Governing Law:

• This Agreement shall be governed by and construed in accordance with the laws of [New York/Brooklyn].
